Here we demonstrate that palbociclib-mediated inhibition of E2F/c-myc hinders glucose metabolism in TNBC cells, by inhibiting GLUT-1 expression and glucose uptake under both normoxic and hypoxic conditions. c-Myccooperates with HIF-1 to induce the expression of glycolytic enzymes [33]; interestingly, palbociclib inhibited also hypoxia-induced HIF-1α accumulation, which might contribute to reduce glucose utilization under hypoxia. This result is in line with a recent study showing that palbociclib destabilizes HIF-1α in colon cancer cells under either normoxic or hypoxic conditions [34]. In our study, the inhibitory effects of palbociclib on glucose metabolism were further enhanced by the combination with PI3K/mTOR inhibitors, being the AKT/mTOR signaling another key player in cancer metabolic reprogramming [35]. Accordingly, palbociclib combined with the mTOR inhibitor everolimus has been recently demonstrated to inhibit aerobic glycolysis in glioblastoma cells [36]. Importantly, we observed a strong impairment of glucose metabolism when the combination with palbociclib and PI3K/mTOR inhibitors was preceded by treatment with palbociclib alone, likely contributing to the synergistic anti-tumor effects associated with this schedule of treatment.

There are several reasons that may explain discrepancies in study outcomes. These include smaller samples sizes with few and/or no subjects in one genotype [75, 273, 274], as well as shorter distances or different types of performance test (power versus endurance) [75] compared to the aforementioned trials, which reported improved endurance after caffeine ingestion in those with the CYP1A2 AA genotype [208, 263]. The effects of genotype on performance might be the most prominent during training or competition of longer duration or an accumulation of fatigue (aerobic or muscular endurance) [149], where caffeine appears to provide its greatest benefits, and where the adverse effects to slow metabolizers are more likely to manifest [195, 260]. Indeed, in a study of performance in elite basketball players [240], only in those with the AA genotype caffeine improved repeated jumps which requires maintaining velocity at take-off repeatedly as an athlete fatigues throughout a game (muscular endurance) - even though there was no caffeine-genotype interaction effect for this outcome. However, caffeine similarly improved performance in those with the both AA and C-genotypes during a simulated basketball game [240]. In a cross-over design of 30 resistance-trained men, caffeine ingestion resulted in a higher number of repetitions in repeated sets of three different exercises, and for total repetitions in all resistance exercises combined, which resulted in a greater volume of work compared to placebo conditions, but only in those with the CYP1A2 AA genotype [262]. Although more research is warranted, there is a growing body of evidence to support the role of CYP1A2 in modifying the effects of caffeine ingestion on aerobic or muscular endurance-type exercise, which helps to determine which athletes are most likely to benefit from caffeine.

One hypothesis by which this occurs is via microbial proteins that mimic tumor antigens, resulting in T cell cross-reactivity; T cells may be primed by microbial antigens in the gut and travel to tumor sites, or the gut microbes or microbial antigens themselves may translocate to distant sites to induce a local T cell response near the tumor [40]. A second hypothesis is that gut microbes or microbial products activate pattern recognition receptors (PRRs), which stimulate the production of cytokines and interferons, thus leading to immunostimulatory or immunosuppressive reactions in T cells; PRR activation may occur in the gut and stimulate traveling innate immune cells, or the microbes or microbial products themselves may translocate [40]. Evidence for these hypothetical mechanisms underlines the potential causal impact of the gut microbiota on immunotherapy response and supports the future use of microbiome manipulation to increase the efficacy of immunotherapy [40].
